2. TELEPHONE CALL: The Therapy Services Coordinator will coordinate a day and an hour to meet for an initial assessment to review your application form and gather more client information.
After review, it will be decided if client can proceed to initial assessment. Based on availability and suitability for programming.
3. INITIAL ASSESSMENT: Your treating therapist will conduct an initial evaluation. Together with the client to establish client goals and create a plan of care.
Stable Connections is a private pay facility that does not bill directly to insurance. However, we do provide documentation to assist in the reimbursement process.
